Navigating Chronic Illness: Why Your Symptoms Can Be Real Even When Tests Are Normal

What the Virtual Sjögren's Summit taught us about chronic illness, autoimmune disease, the nervous system, and why bringing medical understanding into psychological spaces—and psychological understanding into medical spaces—may be one of the most important developments in modern healthcare.

This article explains why people living with chronic illness or autoimmune disease may experience significant symptoms even when standard medical tests appear normal. Drawing on presentations from the 2026 Virtual Sjögren's Summit, it explores interactions between the immune system, nervous system, metabolism and psychological wellbeing. It also discusses medical gaslighting, patient advocacy, nervous system regulation, pacing and the importance of integrating psychological support with medical care. The article argues that psychology does not replace medicine and medicine does not replace psychology, and explains how counselling can help people navigate chronic illness, healthcare relationships, uncertainty and the psychological impact of serious or chronic medical conditions.
Author's note: Much of the educational information in this article is based on presentations delivered during the 5th Virtual Sjögren's Summit (2026). Although several presentations focused specifically on Sjögren's disease (Sjögren's syndrome), many of the biological and psychological principles discussed apply to a wide range of autoimmune diseases and chronic illnesses. References to the summit presenters are included at the end of this article.
Why many people with chronic illness feel unseen
One of the strongest messages repeated throughout the Virtual Sjögren's Summit was remarkably simple:
Your symptoms are real, even when your tests are normal.
For many people living with a chronic illness or autoimmune disease, this sentence carries enormous emotional weight.
Perhaps you have experienced persistent fatigue, brain fog, widespread pain, digestive problems, dizziness, dry eyes, dry mouth, headaches, poor sleep, or difficulty concentrating. Yet each new blood test comes back "normal." Imaging studies reveal little. One specialist refers you to another. Eventually, you begin wondering whether anyone—including yourself—should believe what your body is telling you.
This experience is not unique to people living with Sjögren's disease. It is shared by many people living with autoimmune diseases, invisible illnesses and medically complex conditions.
During the summit, several presenters described the common journey of being passed from one specialist to another while each clinician focuses on a single body system. A rheumatologist examines the joints. A gastroenterologist investigates the digestive tract. A neurologist looks at headaches or numbness. An ophthalmologist treats dry eyes. Each specialist may be doing excellent work within their own field, yet no one is looking at the whole picture.
As a result, people often leave with multiple diagnoses—or no diagnosis at all—but still without an explanation that connects their symptoms into a coherent story.
This gap between lived experience and medical findings has consequences that extend far beyond physical health.
Many people begin doubting themselves.
Others stop mentioning symptoms because previous concerns were dismissed.
Some withdraw socially because explaining an invisible illness becomes exhausting.
Many eventually experience what has become known as medical gaslighting—the feeling that genuine physical symptoms are repeatedly minimised, dismissed or attributed solely to stress, anxiety or personality.
The summit presenters were careful to distinguish between uncertainty and dismissal. Medicine does not yet have answers for every symptom, but uncertainty should never invalidate a person's lived experience.
That distinction matters enormously.
Feeling believed is not simply emotionally comforting—it often becomes the foundation for rebuilding trust in healthcare and beginning the long process of learning how to navigate chronic illness.
Understanding the "Perfect Storm"
One framework presented during the summit described chronic illness as a "Perfect Storm", involving several interacting biological systems.
Rather than searching for one root cause, one food intolerance, one abnormal blood test or one miracle treatment, the presenters encouraged participants to think about multiple biological systems interacting simultaneously.
Imagine several weather fronts colliding.
Each one contributes to the overall storm.
No single raindrop causes the thunderstorm.
Likewise, many chronic illnesses develop through the interaction of multiple biological systems.
The summit identified four major "weather systems":
1. Barrier disruption
Our bodies contain protective barriers that separate us from the outside world, including the skin, respiratory tract, digestive tract and other specialised tissues.
When these barriers become more permeable than intended, the immune system may begin reacting more strongly to substances that would normally be tolerated.
2. Dysbiosis
The human microbiome—the trillions of microorganisms living on and inside us—plays an important role in regulating immune function.
Changes in these microbial communities, whether through illness, antibiotics, stress, diet or other influences, may contribute to immune dysregulation.
3. Immune and nervous system dysregulation
One of the most fascinating themes throughout the summit was the increasingly recognised communication between the immune system and the nervous system.
Rather than functioning independently, these systems continuously exchange information.
When one becomes dysregulated, the other often follows.
4. Metabolic and hormonal function
The presenters also highlighted the importance of cellular energy production, hormonal regulation and metabolic processes.
Viewed through this lens, symptoms such as fatigue become more understandable.
Fatigue is not necessarily evidence of laziness, lack of motivation or poor resilience.
Instead, it may reflect the enormous energy demands placed on a body attempting to regulate multiple dysregulated systems simultaneously.
The value of this framework is not that it explains every chronic illness perfectly.
Its value lies in shifting attention away from searching endlessly for one missing puzzle piece and instead recognising that complex illnesses often emerge from interactions between many biological processes.
The nervous system is not "all in your head"
Perhaps the most significant scientific theme running throughout the Virtual Sjögren's Summit was the growing understanding of the relationship between the immune system and the nervous system.
Historically, medicine has tended to separate these systems into different specialties.
Neurologists treat the nervous system.
Rheumatologists treat autoimmune disease.
Psychologists address emotional wellbeing.
Endocrinologists manage hormones.
Yet our bodies do not divide themselves into specialties.
Throughout every moment of every day, these systems communicate continuously through complex biological feedback loops.
One concept discussed repeatedly was that inflammation can directly influence the brain and nervous system.
This helps explain why many people living with chronic illness experience symptoms such as:
Brain fog
Mental fatigue
Poor concentration
Changes in mood
Disturbed sleep
Heightened pain sensitivity
Importantly, this does not mean these symptoms are "psychological" in the dismissive sense of the word.
Quite the opposite.
The summit presenters repeatedly emphasised that these experiences have genuine biological foundations.
Similarly, nervous system dysregulation does not imply that stress caused the illness.
Rather, once an autoimmune disease or chronic inflammatory condition develops, the nervous system and immune system may begin reinforcing one another.
The nervous system remains on high alert.
The immune system remains activated.
Each system influences the other.
Understanding this relationship represents a major shift in how many researchers now think about chronic illness.
Instead of asking whether symptoms are "physical" or "psychological," the more useful question becomes:
How are these biological systems interacting with one another, and how might we support them more effectively?
Why symptoms change from one day to the next
One of the most frustrating aspects of living with a chronic illness is unpredictability.
Some mornings you wake feeling reasonably well.
The following day, after doing what appears to be exactly the same activities, you struggle to get out of bed.
To outsiders, this inconsistency can appear confusing.
To the person living with the illness, it can be deeply discouraging.
Several summit presenters explained this using the idea of biological feedback loops.
Stress influences the immune system.
The immune system influences the nervous system.
The nervous system influences sleep.
Poor sleep increases inflammation.
Increasing inflammation contributes to pain, fatigue and cognitive difficulties.
Those symptoms create additional stress.
The cycle continues.
Recognising these loops changes how we think about symptom flares.
Instead of asking,
"What single thing caused today's flare?"
we begin asking,
"Which combination of biological and environmental factors may have nudged my system beyond its current capacity?"
Poor sleep.
A viral illness.
Travel.
Emotional stress.
Hormonal changes.
Heat.
Overexertion.
Several of these occurring together may be enough to push an already sensitised system beyond its threshold.
This perspective is both scientifically and psychologically significant.
It replaces self-blame with curiosity.
Instead of seeing flare-ups as personal failures, they become opportunities to better understand how our own bodies respond under different circumstances.
For many people navigating chronic illness, autoimmune disease, or Sjögren's disease, that shift alone can be profoundly freeing.
Rather than constantly fighting against the body, we begin learning how to work with it.
Navigating Chronic Illness: Connecting Psychological and Medical Support
Understanding chronic illness requires us to connect the dots between the physical and psychological aspects of what a person is experiencing. A model in which every medical episode is assumed to have a single cause is often too simplistic to reflect the reality of living with a chronic condition. As the Virtual Sjögren's Summit demonstrated, symptoms can emerge from complex interactions between immune, nervous, hormonal and other biological systems. At the same time, the experience of living with those symptoms inevitably has psychological and emotional consequences.
An exclusively psychological view of chronic medical conditions can be equally problematic. If physical symptoms are reduced to anxiety, stress or mindset, the condition itself can effectively be turned into a psychological problem. This can contribute to patient-blaming and medical gaslighting: the suggestion that you simply need to try harder, exercise more willpower, change your mindset or think more positively. When important medical factors are excluded, patients can be set up to fail—and may end up blaming themselves when their symptoms persist.
An exclusively medical view has its own limitations. Focusing only on the biological disease can overlook the psychological effects of living with chronic symptoms, uncertainty, disability, loss and changes to identity and everyday life. It can also mean that patients miss out on psychological interventions that can help them cope, adapt, communicate, build resilience and support their medical treatment.
Psychology does not replace medicine, and medicine does not replace psychology.
Both are essential. And, as the summit presentations highlighted, the relationship between them is not merely theoretical. The body and mind are constantly communicating.
This raises a more useful question:
How can we support this relationship more effectively?
This is where concepts discussed throughout the summit—such as nervous system regulation, pacing, developing resilience and building sustainable routines—become relevant. These approaches are not alternatives to appropriate medical treatment. Rather, they can form part of the broader support a person may need while living with a chronic or serious medical condition.
Moving beyond one-size-fits-all chronic illness support
There is, however, an important trap to avoid.
Once we discover that a particular strategy can be helpful, it is tempting to turn it into a universal prescription. Online resources are full of advice about breathing exercises, diets, supplements, exercise, sleep routines, mindfulness and other approaches to managing chronic illness. Some of this information may be useful. But generic advice has an inherent limitation: it isn't designed specifically for you.
Your illness does not exist in isolation from the rest of your life.
Your symptoms, medical history, personality, relationships, responsibilities, resources, previous experiences and current circumstances all influence what is realistic and helpful for you.
This is why a personalised approach to navigating chronic illness matters.
In my counselling work, I regard you as the expert on your own experience. My role is not to arrive with a predetermined set of solutions, but to listen, ask questions, help you identify patterns and draw on your existing resources, skills and strengths. Together, we can explore what may be useful in your particular circumstances and develop approaches that are realistic enough to use in everyday life.
A tailored approach is more likely to be sustainable than trying to force yourself into someone else's formula for managing chronic illness.
Psychological support can also help you navigate the medical system
The connection between psychology and medicine extends beyond what happens inside your own body.
Sometimes, navigating chronic illness also means learning how to navigate the healthcare system itself.
Finding the appropriate specialist, understanding which services may be available, preparing for an appointment, explaining your symptoms clearly and knowing which questions to ask can make a significant difference to the quality of a medical consultation.
Unfortunately, not every medical interaction feels genuinely patient-centred. Some healthcare professionals may still struggle to communicate in a way that recognises the patient as a whole person rather than simply the condition being treated. When symptoms are complex or difficult to measure, patients may also find themselves having to advocate for themselves.
Patient advocacy does not necessarily mean becoming confrontational or challenging every medical recommendation. Often, it means becoming sufficiently informed and confident to participate actively in your own care.
For example, psychological support can help you:
clarify what you most need from a medical appointment;
organise and communicate your symptoms and concerns;
identify the questions you want to ask;
prepare for difficult conversations with healthcare professionals;
cope with uncertainty when there are no immediate answers;
recognise when you may need to seek another opinion;
process the emotional impact of being dismissed or misunderstood; and
develop greater confidence in participating in decisions about your care.
A counsellor cannot diagnose your medical condition or replace your doctor. But counselling can help you develop the psychological skills and confidence needed to navigate the medical system more effectively.
Bringing psychological insight into medical spaces—and medical understanding into psychological spaces
This is ultimately why the connection between medicine and psychology matters so much to me.
When you are living with a chronic or serious medical condition, you shouldn't have to choose between understanding the biology of your illness and receiving psychological support for the experience of living with it.
The two perspectives can—and should—inform one another.
The medical understanding helps us appreciate what your body may be experiencing.
Psychological understanding helps us appreciate what it is like to live inside that body: the uncertainty, fear, frustration, grief, identity changes, relationship challenges and practical decisions that accompany chronic illness.
The goal isn't to find a psychological explanation for a medical condition.
Nor is it to ignore the psychological dimensions of illness because the condition is "physical."
The goal is to see the whole person.
That is why I constantly strive to bring psychological insight into medical spaces and medical understanding into psychological spaces.
For people living with Sjögren's disease, another autoimmune disease, or a serious or chronic medical condition, this integrated perspective can provide a more compassionate and practical way of navigating illness—one that takes both the biology of the condition and the person experiencing it seriously.
